Request for Transportation to a Community, Non-Public, or Parochial School
2021-2022 School Year

State law requires public schools to provide transportation to community, non-public, and parochial school students according to the following guidelines:

  1. Transportation must be requested of the public school district by the parent; and
  2. Transportation must be provided according to the same guidelines as those established for resident students attending public schools in the district.

The Warren City Schools Board of Education Policy states: Children living beyond the following walking limits shall be entitled to bus transportation:

  1. Kindergarten through 8th Grade, 1 mile
  2. Grades 9th through 12th Grade, 2 miles

The Warren City School District’s Transportation Department will contact you regarding your request using the information provided on this form.
For your student’s safety, DO NOT put your student on a Warren City Schools’ bus until you have been notified by Transportation.
